Register quickly and easily
M3 Mondzorg is happy to welcome new patients! Whether you live in the Netherlands or abroad.
Once we have received your registration (and that of your family members or partner), our receptionist will contact you by phone to schedule your first appointment with the dentist. Your first appointment will take place within a few weeks. As a patient of M3 Mondzorg, you can also always use our emergency service outside of office hours, on weekends, or during holidays.
Health Questionnaire
If you are 18 years or older, we would also like to receive a completed health questionnaire from you to complete your registration. Once we have both forms returned, we will contact you by phone to schedule your first appointment. This appointment will take place within a few weeks.
We kindly ask you to fill out both forms (health questionnaire: for those who are 18 years or older) and return them.
Regarding your payment at M3 Mondzorg
If you visit the dentist at least once a year and we receive your dental record from your previous dentist prior to your first appointment at M3 Mondzorg, you will receive your invoice through Infomedics after your check-up or treatments at M3 Mondzorg. If not, we would like you to pay the expenses of your treatment at our reception directly after your visit.

Become a patient?
At all of our locations, you can receive comprehensive dental care. Our treatments are all aimed at keeping your teeth in optimal condition. We prioritize prevention, and you will receive an invitation for a dental check-up every six months. In addition to regular care, we also offer treatments in the fields of implantology, orthodontics, crowns and bridges, dentures, implant-supported dentures, whitening treatments, and aesthetic dentistry such as veneers. All of this is supported by our own dental laboratory and the M3 dental hygiene team.
